This website is not a clinical record. Therapy notes and other clinical records are kept in the telehealth platforms we use (Plushcare, Alma, and Rula). They are not kept in these website forms.
Website forms are not for clinical communication and not for emergencies.

Who we are
marriageandfamily.com is the public site for this practice.
The practice is operated by Spurlock Medical Corporation, a California corporation. Mark Spurlock, LMFT is Vice President. He is a California Licensed Marriage and Family Therapist, license 45318.
We offer video counseling for adults in California: Christian husband-and-wife marriage work, and individual adult therapy. We do not see minors. We do not see people who are outside California. We do not handle emergencies on this site or by website form.

What this site collects
Forms
When you fill out a form on this site, we may collect:
- First name
- Last name
- Phone
- A message, if that form has a message box
Most forms ask for first name, last name, email, and phone. The workbook / free-guide form may later drop last name and phone and keep a shorter set of fields.

Like most sites, ours may collect ordinary technical data, such as:

This website is not a clinical record. A form on marriageandfamily.com is not your chart. Do not put diagnosis details, session history, or other clinical information in a website form. Those forms are not how we keep therapy records.
Therapy records are kept in the telehealth platforms (Plushcare, Alma, and Rula), not in these forms.
If you already sent clinical details through a website form by mistake, email info@marriageandfamily.com and we will handle it as carefully as we can. That still does not turn the website into your chart.
